Body orientation: anatomical position, planes, directions, cavities and regions
๐งญ Every A&P question is written in a shared map language. Learn the map once and "distal," "sagittal" and "left lower quadrant" stop being roadblocks.
๐ก Key idea
Directions are always described from anatomical position, using the patient's own left and right.
๐ง The idea
All directions assume ANATOMICAL POSITION: standing upright, facing forward, arms at the sides, palms forward, feet slightly apart. Left and right mean the patient's left and right. PLANES slice the body: the sagittal plane divides left from right (midsagittal = equal halves), the frontal (coronal) plane divides front from back, and the transverse (horizontal) plane divides top from bottom. DIRECTIONAL TERMS come in pairs: superior/inferior (above/below), anterior or ventral/posterior or dorsal (front/back), medial/lateral (toward/away from the midline), proximal/distal (closer to/farther from where a limb attaches), superficial/deep. BODY CAVITIES: the dorsal cavity holds the cranial cavity (brain) and vertebral canal (spinal cord); the ventral cavity holds the thoracic cavity (lungs, heart in the mediastinum) separated by the diaphragm from the abdominopelvic cavity. The abdomen is mapped by four QUADRANTS (right upper, left upper, right lower, left lower) or nine regions (e.g., epigastric, umbilical, hypogastric).
โ๏ธ Worked example
"The wrist is ___ to the elbow." Both are on the upper limb, so use proximal/distal. The limb attaches at the shoulder; the wrist is farther from it, so the wrist is DISTAL to the elbow. Next: "The sternum is ___ to the lungs." Breastbone is toward the midline and the lungs sit to each side, so the sternum is MEDIAL. Last: an appendix pain point lies to the patient's right, below the navel, so it falls in the RIGHT LOWER QUADRANT.
๐ Takeaways
- Sagittal = left/right, frontal (coronal) = front/back, transverse = top/bottom.
- Proximal/distal are for limbs; medial/lateral are relative to the midline.
- The diaphragm separates the thoracic cavity from the abdominopelvic cavity.
๐ Terms in this lesson
Sagittal plane: A vertical plane that divides the body into left and right parts (the midsagittal plane makes equal halves); for example, a cut down the midline between the eyes.
Frontal (coronal) plane: A vertical plane that divides the body into front (anterior) and back (posterior) parts; for example, a cut from ear to ear down through the shoulders.
Transverse plane: A horizontal plane that divides the body into upper (superior) and lower (inferior) parts; for example, a cross-section at the waist.
Proximal and distal: Proximal means closer to where a limb attaches to the trunk, and distal means farther away; for example, the elbow is proximal to the wrist.
Medial and lateral: Medial means toward the body's midline, and lateral means away from it; for example, the nose is medial to the eyes.

Question 1 of 3
Which plane divides the body into anterior and posterior portions?
- Frontal plane
- Midsagittal plane
- Sagittal plane
- Transverse plane
Show the answer and rationale
Answer: A
The frontal (coronal) plane runs side to side and separates front from back. A sagittal or midsagittal plane separates left from right, and a transverse plane separates upper from lower.
Question 2 of 3
In anatomical position, the thumb is located on which side of the hand?
- Medial
- Lateral
- Proximal
- Posterior
Show the answer and rationale
Answer: B
With palms facing forward, the thumb points away from the midline, so it is lateral. The little finger is medial. Proximal describes nearness to a limb's attachment, not side of the hand.
Question 3 of 3
The heart lies within which subdivision of the thoracic cavity?
- Pleural cavity
- Mediastinum
- Vertebral canal
- Pelvic cavity
Show the answer and rationale
Answer: B
The mediastinum is the central compartment of the thoracic cavity and holds the heart, great vessels, esophagus and trachea. Each lung sits in its own pleural cavity; the vertebral canal is part of the dorsal cavity.
